When did this happen and what is the story behind the 2 stars not being on the World Scout patch on the new uniforms?

The stars not being there and the knot being wrong are an error

Jessica posted about it in the Facebook group in the Uniform Q&A doc

12. Q- why is the new Wold Scouting Badge missing the 2 stars on the Fleur de Leaf?
12. A (by Jessica) This is the result of a very unfortunate production error which was not caught until shirts were already in stores. We really, really regret this oversight. Replacement badges are available in Scout shops which can be placed over the incorrect emblem. This error will be corrected in future runs.

12a) Q Is this badge being 'given' with the purchase of the uniform shirt (if we have bought a shirt with the wrong emblem we can go get it)? or is it an additional cost (if we want the right emblem we have to buy it)?
12a) A (by Jessica) Replacement emblems are available free of charge at Scout shops. Contact your local shop for details if you have already purchased a shirt.

If you go to your Scout Shop they can give you a badge for you to sew over the embroidered World Scout Badge. (or iron on, though not sure if the iron on will work over the embroidery that is already in the shirt)
